If she likes it when he goes down on her I don't understand the discussion about asexuality. People, sex is more than a penis inside a vagina. For some people the idea of penetration is not the most attractive or easy to digest, especially before having done it a good amount of times.
Her sister got pregnant in high school, that may have been quite traumatic and she may have linked the idea of penetrative sex to danger or negative outcomes and it takes time and positive exposure to change that belief.
Also, just the fact of not having penetrative sex for so much of her adult life in a world that is so obsessed with it can make it seem like a HUGE deal. The combination of penetration not being inherently pleasurable for most women (especially the first few times) and the taboo she may feel around it is enough to make anyone anxious.
Not saying there's nothing else to the story but it may be simpler too.
