I'm a doctor, and about 50% of the time I ask this in the office, I get an automatic 'Fine, thanks. How are you?' Which is hilarious and sets me up for my favorite joke of 'OK, well, I guess we can wrap up this visit then' while doing the Midwestern thigh slap

Sometimes, when I'm feeling salty, people ask, "How are you?" I answer, "Why? What did you hear?"

My least favorite is: "Howya doon." It's not a question.
