The Minecraft mod RedPower2 came with a FORTH interpreter - that's the first, last, and only time I've ever used it. I thought it was a pretty cool addition to the game in 2013.

I've written two forth-like languages. I tend to agree with other poster(s): once I build it I tend to not use it much (and in my case I go back to the scheme-like language I wrote-which I find much more functional/ergonomic for actual work). That said, forths are fun to implement and are easy to work through odd ideas and designs with.
