tl;dr

This raises awareness that the client usually oparate in her own aspect
of reality and your job is to, sometimes silently, lift the problem out
from ambiguity.

longer boring sterile example.. sort of -

I was recently asked to run a query for an entity of type A. I did. In
fact, the data model is such that entities of type A can be associated
with multiple entities of type B (one-to-many relationship).

This query turned into a quasi-standard, copied around and used.

In fact, it turned out that "a query for entity of type A" was really
only for a special entity B associated, not expressed as special in the
data model. The one entity of type A, for which the original query was
requested, had only that special entity of type B associated.
