Significant fuck-ups are quite rare. People are able to cognitively know that yelling "gas the jews" is probably a bad idea. Repeated small errors (ex. useless or damaging habits) are more likely and people are not aware of them so they do not fix them.

TBD there are ways of hacking habits. Scott Adams mentioned one (tying new habits to old ones.) Other resources exist for these (ex. tracking charts to make sure something happens for 30-90 days in a row.)
