I think this bot is pretty neat, the hate you get is undeserved! I think a good medium of asking for permission to post would be, if the bot is going to post in a subreddit for the first time, it instead sends a modmail to that subreddit asking for permission. A reply of (for example) 'deny' and it will blacklist the sub. An 'accept' or no reply after 24-48 hours, then it starts posting. They can reply to the modmail in the future to change it.

Put a link to this subreddit in the bot's comment.
EDIT: Also set a way for a user to blacklist himself with a PM. A link to the PM template could be put in bot's comments.
However this would flood the inbox, which is bad because you are using it for feedback. The subreddit idea above could solve that problem

Do you actually track how many people use your spam? Bots like this add a ton of visual noise to reddit, and are pretty unpopular. Do you have any data that suggests that people actually appreciate this bot? It's annoying as hell and the feedback in this subreddit seems to bear that out. At best, you save a small percentage of people exactly two clicks and you spam people's inboxes and threads.
I would be interested in trying to understand your thought process in creating this and releasing it into the wild and thinking that it's a net positive for the site. And based on how you promised opt-in and then said you're not really interested in maintaining the bot, why not just take it offline? If you are providing a service to anyone (which is not something I'm willing to admit), you're paying for it by annoying the bejesus out of how many other people?
So what are you thinking that this is a good idea? Or do you just not care? You made it and it's on reddit and fuck anyone who has a problem with it?
The fact that you're hiding your real account behind the bot speaks a lot for what you really think about its quality and purpose too.

I cannot imagine how can you handle all those extremely negative responses. I'm even tilted right now.
It's a neat bot, I keep seeing it but it actually makes me check those subs quickly. Spammy? Maybe, not for me though. At least you're acting accordingly to the feedback(!) and doing your best to make your bot better.
