Much of this info was collected through r/MarchAgainstTrump. When they first started that sub I put together a warning post showing how the people collecting info through the sub were affiliated with the alt-right subs. I got suspended for "doxxing" after I put up the warning.

The Centipede Central server was created by members of the /r/The_Donald subreddit.
This discord is listed on the sidebar of /r/the_donald and run by the mods.

Well that's more 4chan starting it, and donald passing it around.
There's certainly an overlap between the two group, and in all likelihood, it being passed on the donald's discord, prob led to a bunch of that sub joining in on the doxing, but that's not really something we can go and blame the sub on as creating. I doubt the mods will officially endorse that document, but I wouldn't be surprised if they spread it around on alt accounts.

Sensationalized headline and a really awful article. /pol/ made a list and extended it, then a single user posts it in an off-site chatroom once, with the screenshot of the log cutting off right below it instead of having it at the top. Maybe the reactions weren't positive in there? Said user also doesn't exist on Reddit. I've also taken a look at their Discord server, it doesn't even require you to register before chatting, and they've said that there wasn't really any reaction to the link being posted (take that as you will).
It just isn't enough to make some grand judgment on T_D.
