To be honest I'm not sure, I haven't dug into it, but I have seen it happen in a few places. I suspect that a reply was sent without including the "in-reply-to" header, but I'm not sure under what conditions that happens. There is one more issue that the topics are listed by creation date, rather than getting bumped up by last activity date, but I'm fine with it as-is for browsing through historical posts, especially because the code is surprisingly small and simple (only 55 lines of Python code!).
