How did they express emphasis on typewriters? On the other hand, the output of typewriters wasn't meant to be accessible to screen readers. I have already listed a bunch of different ways to convey the same thing as you would with italics and their drawbacks. Sidenote: Since gemtext is UTF-8 by default and lack a way to suggest a custom font, you could probably extend UTF-8 in the range <U+F0000> to <U+10FFFF> for markup, like <U+F7777> for emphasis.
