It should be rare for hashtags to appear at the beginning of a line (even in your example they didn't), but in such a case there could indeed be a conflict. In such cases perhaps something like \# could be used to start a line. The space idea also seems good to me and maybe more efficient. Personally I always space my headers as you suggested.
